Reader API

Allow adding a dedicated source/origin field to API-created documents, returned in responses and usable as a filter in Reader instead of relying on tags.

Allow updating the body/content of an existing saved document via the API without creating a new document, while preserving the original saved date. This would support use cases like correcting minor errors in otherwise stable content.
